This has been a long running story in Adelaide, Australia, where a number of chemotherapy patients were administered incorrect dosages due to a computer error, some dating back 2 years.
Now a senior bureaucrat has been forced to defend the lengthy delays in initiating a review into the issue.
"Dr Sherbon – who described the radiation blunder in which 869 Royal
Adelaide Hospital cancer patients were underdosed with radiotherapy as
a "near miss" – told a budget and finance committee yesterday that it
was "somewhat of a leap" to conclude that every cancer treatment system
in the state should have been checked after the mistake came to light."
"In a second cancer treatment bungle, it was revealed last week that 11
children suffering cancer were overdosed with chemotherapy drugs at the
Women's and Children's Hospital as a result of human and computer error."
